Roman Chamomile (Chamaemelum nobile) has a long history of traditional use in European herbal medicine for the relief of cramps and muscle spasms, particularly those associated with menstrual discomfort and digestive issues. The herb has been used in the form of teas, tinctures, and topical applications. The traditional rationale is based on its reputed antispasmodic and calming effects, which are attributed to its essential oil components such as apigenin, bisabolol, and chamazulene. These compounds are believed to act on smooth muscle to reduce spasms and promote relaxation.

However, scientific validation supporting these effects is limited. Most of the evidence consists of in vitro (test tube) and animal studies, which suggest that chamomile extracts can produce muscle-relaxant effects, possibly by modulating calcium channels and inhibiting smooth muscle contractions. Clinical trials in humans specifically evaluating Roman Chamomile for cramps and spasms are scarce, and most available studies focus on German Chamomile (Matricaria chamomilla), which is closely related but not identical. Therefore, while traditional use is strong and some pharmacological plausibility exists, high-quality clinical evidence is lacking, resulting in a moderate evidence rating.
