Butyrate, a short-chain fatty acid (SCFA), is produced naturally in the colon through bacterial fermentation of dietary fiber and has garnered scientific interest due to its role in maintaining intestinal health. In Crohn's Disease (CD), a chronic inflammatory bowel disease, butyrate levels are often reduced, and the intestinal barrier is compromised. Butyrate is known to support mucosal healing, modulate the immune response, and enhance the function of the epithelial barrier. Several small-scale clinical studies and animal models have investigated the therapeutic benefits of butyrate supplementation for inflammatory bowel diseases, including Crohn's. Butyrate triglyceride is a prodrug formulation designed to deliver butyrate to the colon more efficiently, overcoming the unpleasant odor and rapid absorption issues associated with free butyrate. Despite biological plausibility and some positive preclinical and early clinical data, the evidence base remains limited. Most studies are small, open-label, or pilot in nature, and results are mixed regarding clinical remission and mucosal healing. There are no large, well-powered randomized controlled trials conclusively demonstrating efficacy for butyrate triglyceride in Crohn's Disease. However, some patients have reported symptomatic improvement, and butyrate's anti-inflammatory and barrier-protective effects provide a rationale for further research. Thus, while the use of butyrate triglyceride in Crohn's Disease is supported by scientific rationale and preliminary studies, the overall evidence quality is low to moderate, justifying a score of 2.
