Fructo-oligosaccharides (FOS) are short-chain carbohydrates classified as prebiotics, meaning they are non-digestible food ingredients that stimulate the growth and/or activity of beneficial gut bacteria. The rationale for their use in Crohn's Disease is based on the hypothesis that modulating the gut microbiome may improve disease symptoms or reduce inflammation, as Crohn’s is associated with dysbiosis (an imbalance in gut bacteria).

Some small clinical studies have investigated FOS supplementation in Crohn’s Disease. For example, a 2006 randomized controlled trial (Benjamin et al., Gut 2006) administered FOS to patients with active Crohn’s disease and found an increase in beneficial bifidobacteria; however, there was no significant improvement in clinical disease activity compared to placebo. Other studies have shown that prebiotics, including FOS, may have modest effects on markers of inflammation or immune function, but consistent improvements in symptoms or disease remission have not been demonstrated.

Overall, the scientific evidence supporting FOS for Crohn’s Disease is limited and inconsistent. While there is a plausible mechanism and some preliminary data suggesting prebiotics could influence gut microbiota and immune responses, larger and more rigorous clinical trials are needed to establish clear benefits for Crohn’s Disease management. Thus, FOS use for this condition is not broadly recommended in clinical guidelines at this time.
