Hyperimmune milk peptides (HMP), also known as "immune milk" or "bovine colostrum enriched with specific antibodies," have been investigated as a nutritional intervention for inflammatory bowel diseases, including Crohn's disease. The rationale is that milk from immunized cows contains antibodies against intestinal pathogens and immune-modulating peptides, which might help reduce gut inflammation and promote mucosal healing. Several small clinical studies and pilot trials have explored the effect of HMP in Crohn’s disease. Some studies, such as Playford et al., 1999 (Gut), demonstrated modest improvements in gut permeability and inflammatory markers in patients receiving HMP. Another small study (Kelly et al., 2000, Aliment Pharmacol Ther) reported potential symptomatic improvement, but results have not been consistently replicated in larger, well-controlled trials.

Currently, the evidence base is limited, with most studies being small, short-term, and sometimes lacking rigorous controls. There is no strong consensus or guideline recommending HMP as a standard therapy for Crohn’s disease. Therefore, while there is a biological rationale and some early clinical evidence, the overall support is weak to moderate, and HMP should not replace established therapies. More robust, large-scale randomized controlled trials are required to ascertain its efficacy and safety in Crohn’s disease management.
