Beta Hydroxybutyrate (BHB) is a ketone body produced during states of low carbohydrate availability, such as fasting or ketogenic diets. There is growing scientific interest in BHB’s potential to support brain function, particularly in the context of neurodegenerative diseases like dementia and Alzheimer’s disease. The rationale stems from evidence that brain glucose metabolism is impaired in these conditions, while ketone metabolism remains relatively preserved. Some small clinical trials and animal studies have shown that supplementation with exogenous ketones, including BHB, can increase brain ketone levels and may lead to modest improvements in cognitive performance in patients with mild cognitive impairment or early Alzheimer’s disease. However, the number of human clinical studies is limited, sample sizes are small, and results are preliminary. Larger, well-controlled trials are needed to confirm efficacy, determine optimal dosing, and assess long-term safety. Thus, while the use of BHB in dementia is supported by plausible biological mechanisms and early clinical research, the strength of current evidence is low to moderate.
