Butylphthalide is a compound originally isolated from celery seed and developed as a pharmaceutical agent in China, primarily for the treatment of ischemic stroke. There is emerging scientific interest in its potential for treating or supporting dementia, particularly vascular dementia and Alzheimer's disease. Preclinical studies in animal models have shown that butylphthalide may improve cognitive function, reduce neuronal damage, and enhance cerebral blood flow, possibly through mechanisms such as anti-oxidative effects, reduction of amyloid-beta accumulation, and inhibition of neuroinflammation.

Clinical evidence in humans is limited but growing. Some small randomized controlled trials conducted in China suggest that butylphthalide may improve cognitive scores in patients with mild to moderate vascular dementia, especially when used alongside conventional therapies. However, the quality of these studies varies, with limitations such as small sample sizes, short durations, and lack of international validation. There is very limited evidence for its use in Alzheimer's disease, and no major guidelines outside China currently recommend it for dementia treatment.
