Citicoline (also known as CDP-choline) is a naturally occurring compound that serves as an intermediate in the synthesis of phosphatidylcholine, an essential component of cell membranes, particularly in the brain. Its use in dementia, especially vascular dementia and Alzheimer's disease, is supported by some scientific evidence, though the overall quality and consistency of the data are limited.
Several clinical trials and meta-analyses have investigated the effects of citicoline on cognitive function in individuals with different types of dementia. Some studies have reported modest improvements in memory, attention, and global cognitive performance when citicoline is used as an adjunct to standard therapies. For example, a 2015 meta-analysis of randomized controlled trials suggested that citicoline may have beneficial effects on memory and behavior in older adults with cognitive impairment, particularly in vascular dementia. The proposed mechanisms include enhanced phospholipid synthesis, neuroprotection, increased cerebral blood flow, and support of neurotransmitter synthesis (such as acetylcholine).
However, the quality of the evidence is mixed. Some trials show only small or non-significant benefits, and many studies have methodological limitations or small sample sizes. As such, major clinical guidelines do not currently recommend citicoline as a standard treatment for dementia. In summary, while there is some scientific support for its use, the evidence is not strong enough to warrant routine clinical use, and further high-quality research is needed.
