Gastrodin, a bioactive compound extracted from the traditional Chinese medicinal herb Gastrodia elata, has been studied for its potential neuroprotective effects, including applications in dementia and cognitive impairment. Several preclinical studies (in vitro and in animal models) have shown that gastrodin can exert multiple beneficial effects relevant to dementia, such as reducing oxidative stress, modulating neurotransmitter levels, decreasing neuroinflammation, and preventing neuronal apoptosis. Some studies suggest gastrodin may improve cognitive function and memory in animal models of Alzheimer's disease and vascular dementia by inhibiting amyloid-beta aggregation and tau phosphorylation, both key features of Alzheimer’s pathology.

However, clinical evidence in humans is limited. A few small-scale randomized controlled trials and observational studies conducted in China have explored gastrodin’s effects in patients with mild to moderate dementia or cognitive impairment. These studies report modest improvements in cognitive scores, but methodological limitations (e.g., small sample sizes, short duration, lack of blinding) reduce the reliability of these findings. Thus, while there is a scientific rationale and promising preliminary data, robust clinical proof is lacking. Gastrodin is used in China as an approved treatment for various neurological disorders, but its use for dementia outside East Asia remains experimental. Current evidence supports gastrodin as a potential adjunct, but not a primary therapy, for dementia.
