Panax ginseng has a long history of traditional use as a general cognitive enhancer and tonic in East Asian medicine, but its application for dementia, particularly Alzheimer’s disease, has also been explored in modern clinical research. Several randomized controlled trials and meta-analyses have investigated the cognitive effects of ginseng in patients with dementia. Some studies report modest improvements in cognitive function and daily living activities in patients with Alzheimer’s disease who received standardized ginseng extracts, with effects sometimes comparable to those seen with approved Alzheimer’s medications, though usually milder and less consistent.

The proposed mechanisms include antioxidant, anti-inflammatory properties, improved cerebral blood flow, and modulation of neurotransmitter systems. The main active constituents, ginsenosides, are thought to play a role in reducing beta-amyloid accumulation and neurotoxicity, which are central to Alzheimer’s pathogenesis. However, the quality and size of available studies are limited. Many trials have small sample sizes, short durations, and variable dosing, making it difficult to draw firm conclusions about efficacy and safety. Systematic reviews generally agree that while there is some evidence for cognitive benefits, it is insufficient to recommend ginseng as a primary treatment for dementia.
