Idebenone is a synthetic analog of coenzyme Q10, developed in the 1980s. It was investigated for its potential neuroprotective and antioxidant properties, with early studies focusing on its possible benefits in neurodegenerative diseases such as Alzheimer's disease and other forms of dementia. The rationale was based on the observation of oxidative stress and mitochondrial dysfunction in the pathogenesis of dementia, and idebenone’s ability to act as an electron carrier in the mitochondrial respiratory chain. However, the scientific evidence supporting the use of idebenone for dementia is weak. Clinical trials conducted in the 1980s and 1990s produced inconsistent and largely unimpressive results. Some small studies suggested marginal improvements in cognitive function, but larger and more rigorous trials generally failed to demonstrate significant clinical benefits in patients with Alzheimer's disease or other dementias. As a result, idebenone has not been approved by major regulatory authorities such as the FDA or EMA for the treatment of dementia. Current clinical guidelines do not recommend idebenone for this indication. In summary, while there is a scientific rationale and some early clinical investigation, the evidence base is insufficient to justify its use for dementia. Its rating is low due to a lack of robust, reproducible, and clinically meaningful outcomes in well-designed studies.
