Oxaloacetate (OAA) is being investigated as a potential treatment for dementia, particularly Alzheimer’s disease, but scientific evidence supporting its efficacy is currently very limited. OAA is a key metabolic intermediate in the Krebs cycle and is thought to play a role in brain energy metabolism. Preclinical studies in animal models of Alzheimer’s disease have shown that OAA supplementation can improve mitochondrial function, increase glucose metabolism, and reduce markers of neuroinflammation and oxidative stress. These findings are promising and form the basis for its experimental use.
Only a handful of small-scale, early-phase clinical trials in humans have been conducted. As of 2024, published clinical data are sparse. One phase 2, double-blind, placebo-controlled study (NCT03791519) investigated OAA supplementation in patients with mild cognitive impairment and mild Alzheimer’s disease. Results have not yet provided conclusive evidence regarding cognitive improvement, and the sample sizes have been small. Therefore, while there is a mechanistic rationale and some preclinical support, robust clinical validation is lacking.
OAA is not a traditional remedy for dementia; its use is based on recent advances in understanding brain metabolism. In summary, oxaloacetate’s use for dementia is supported by experimental and early clinical evidence, but the current level of scientific validation is weak (evidence rating: 1/5). More comprehensive and larger-scale studies are necessary to determine its therapeutic value in dementia.
