Pyritinol (pyridoxine disulfide) is a semi-synthetic derivative of vitamin B6 developed in the 1960s and used in some European countries for cognitive impairment and dementia. Its proposed mechanisms of action include improving cholinergic transmission and enhancing glucose uptake in the brain. Several small clinical trials conducted primarily between the 1970s and 1990s assessed its efficacy in patients with age-related cognitive decline and mild to moderate dementia. Meta-analyses and systematic reviews have found that, while some studies reported modest improvements in cognitive performance, the quality of evidence is generally low due to methodological limitations, small sample sizes, and inconsistent results. Guidelines from major neurological and geriatric associations do not recommend pyritinol for dementia due to insufficient robust evidence. Its use in this context is thus based on limited scientific validation rather than tradition, and it is not widely endorsed outside certain European countries. Modern large-scale, well-controlled studies are lacking, and pyritinol is not approved for dementia treatment in the US or UK. Overall, the scientific basis for its use in dementia is weak, and it should not be considered a first-line therapy.
