Vincamine is a naturally occurring alkaloid derived from the periwinkle plant, Vinca minor. It has been used in several countries, primarily in Europe, as a vasodilator and cerebral metabolic enhancer. The rationale for its use in dementia, particularly vascular dementia, is based on its pharmacological properties: vincamine is thought to increase cerebral blood flow and oxygen utilization in the brain. Several small clinical studies conducted in the 1970s and 1980s suggested modest improvements in cognitive function and memory in elderly patients with cognitive impairment or mild to moderate dementia. However, these studies were often limited by methodological flaws, small sample sizes, and lack of standardization compared to modern clinical trials.

Systematic reviews and meta-analyses are lacking, and large, high-quality randomized controlled trials have not been conducted. Vincamine is not approved by regulatory agencies like the FDA for dementia treatment, and it is not included in major clinical guidelines for the management of Alzheimer’s disease or other dementias. The evidence base is thus weak and outdated, leading to its limited use in current clinical practice. Overall, while there is some scientific basis and historical clinical use for vincamine in supporting dementia, the strength of evidence is low, and its role is not well established in contemporary medicine.
