Vitamin E, including its various forms such as delta-tocopherol, has been studied for its potential role in supporting or treating dementia, particularly Alzheimer’s disease. The interest in vitamin E stems from its antioxidant properties, as oxidative stress is believed to contribute to neuronal damage in dementia. Most research, however, has focused on alpha-tocopherol, the most biologically active form of vitamin E in humans, rather than delta-tocopherol. Several randomized controlled trials (RCTs) have investigated vitamin E supplementation in Alzheimer’s disease. The most notable among them, published in the New England Journal of Medicine (Sano et al., 1997), found that high-dose alpha-tocopherol (2000 IU/day) modestly delayed functional decline in patients with moderate Alzheimer’s, though the benefit was not dramatic and accompanied by some risks, such as increased hemorrhagic stroke at high doses. There is little direct evidence specific to delta-tocopherol for dementia, and most clinical recommendations concern mixed tocopherols or alpha-tocopherol. Meta-analyses of vitamin E for dementia prevention or treatment have yielded inconsistent results, with some studies showing a slight slowing of functional decline and others showing no effect on cognition. Major guidelines do not currently recommend vitamin E as a primary treatment for dementia. Overall, while the rationale for antioxidant therapy is scientifically plausible, the strength of clinical evidence supporting vitamin E (and specifically delta-tocopherol) for dementia is weak to modest at best.
