L-phenylalanine is an essential amino acid that serves as a precursor to several neurotransmitters, including dopamine, norepinephrine, and epinephrine, which are involved in mood regulation. Its use in the support or treatment of depression is based on the hypothesis that increasing the availability of these neurotransmitters may alleviate depressive symptoms. Some early studies, primarily from the 1970s and 1980s, suggested that both L-phenylalanine and its derivative, DL-phenylalanine, may have antidepressant effects, especially in individuals who do not respond to conventional medications. These effects are presumed to be due to enhanced catecholamine synthesis in the brain. However, the scientific evidence supporting the use of L-phenylalanine for depression is limited and of low quality. Most studies have small sample sizes, lack rigorous controls, or have methodological flaws. There have been few, if any, large-scale, well-designed randomized controlled trials in recent decades. Modern clinical guidelines do not recommend L-phenylalanine as a standard treatment for depression, and its use is rare in mainstream psychiatric practice. Moreover, there are safety concerns, especially in individuals with phenylketonuria (PKU) or those taking certain medications. In summary, while there is some scientific rationale and preliminary evidence, the overall support is weak (rated 2/5), and further research is needed to establish efficacy and safety.
