L-Glutathione (GSH) is a tripeptide and a key endogenous antioxidant that helps protect cells from oxidative stress. Diabetic Retinopathy (DR) is characterized by damage to the retinal microvasculature, and oxidative stress is considered a major contributor to its pathogenesis. There is scientific rationale for considering antioxidants like glutathione in the management of DR. Several preclinical studies have demonstrated that glutathione levels are decreased in the retinas of diabetic animals and in the plasma of patients with DR, suggesting that glutathione depletion may worsen oxidative damage in the retina.

Some small clinical studies and reviews have suggested that antioxidant supplementation (including agents that raise glutathione levels, such as N-acetylcysteine or alpha-lipoic acid) may provide some benefit in slowing the progression of DR. However, direct clinical trials assessing the efficacy of L-glutathione supplementation in DR patients are very limited. Most evidence comes from animal studies or extrapolation from broader antioxidant research.

Current scientific consensus does not support glutathione as a primary or approved treatment for diabetic retinopathy, but it is being explored as an adjunctive therapy in ongoing research. The evidence supporting its use is promising but preliminary, justifying a modest rating of 2. Larger, well-controlled clinical trials are needed to establish its efficacy and safety profile for this specific indication.
