Forsythia (Forsythia suspensa) is a well-known herb in Traditional Chinese Medicine (TCM) and is commonly included in classical herbal formulas such as "Yin Qiao San," which is traditionally used for early-stage febrile diseases with sore throat, including those that may resemble diphtheria. The use of forsythia in this context is primarily rooted in historical and traditional practices rather than modern scientific validation. In TCM, forsythia is considered to have "heat-clearing" and "detoxifying" properties, which is why it has been used for conditions characterized by fever, throat swelling, and infection.
However, there is minimal direct scientific evidence demonstrating that forsythia is effective specifically for diphtheria. While some laboratory studies suggest that forsythia extracts may have general antibacterial and anti-inflammatory effects, these findings are not specific to Corynebacterium diphtheriae, the bacterium responsible for diphtheria, nor do they demonstrate clinical efficacy in human cases of the disease. The use of forsythia for diphtheria is thus best understood as a reflection of historical medical approaches prior to the advent of antibiotics and diphtheria vaccination. Modern treatment of diphtheria relies on antitoxin and antibiotics, and forsythia should not be considered a substitute for these evidence-based therapies.
