Ligustilides are a class of phthalide compounds most commonly found in herbs such as Angelica sinensis (Dong Quai) and Ligusticum chuanxiong (Chuanxiong). These herbs have a long history of use in Traditional Chinese Medicine (TCM) for conditions associated with dizziness, such as "blood deficiency" and "wind" symptoms. The traditional rationale is that these herbs "invigorate blood circulation" and "dispel wind," which, according to TCM theory, can help alleviate dizziness and related symptoms. However, modern scientific studies directly evaluating ligustilide for the treatment of dizziness are extremely limited. Some preclinical studies have explored the neuroprotective, vasodilatory, and anti-inflammatory actions of ligustilide, which theoretically could relate to mechanisms underlying dizziness, such as improved cerebral blood flow. Nonetheless, there is a lack of well-controlled clinical trials specifically investigating ligustilide or ligustilide-containing extracts for dizziness in humans. Thus, while traditional use is well-documented and there is some mechanistic plausibility, scientific validation remains weak. The evidence rating reflects the strong traditional use (especially in Chinese medicine) but limited direct scientific evaluation for this particular indication.
