Synopsis

Source of validity: Traditional
Rating (out of 5): 2

Ligustilides are a group of phthalide compounds primarily found in plants such as Ligusticum chuanxiong (Szechuan lovage) and Angelica sinensis (Dong quai). These plants have a long history of use in Traditional Chinese Medicine (TCM) for treating symptoms such as dizziness, headaches, and disorders related to blood circulation. The traditional rationale is based on the concept that these herbs “invigorate blood” and “dispel wind,” which TCM practitioners associate with alleviating dizziness and vertigo.

Scientific studies on ligustilides are limited, and most research focuses on their anti-inflammatory, neuroprotective, and vasodilatory properties in vitro or in animal models. While these effects could theoretically relate to improved cerebral blood flow or reduced vascular spasms, which might help with certain types of dizziness, direct clinical evidence in humans is lacking. There are no high-quality clinical trials specifically demonstrating that ligustilides or ligustilide-containing preparations treat or relieve dizziness.

In summary, the use of ligustilides for dizziness is primarily justified by traditional use in TCM, with only indirect or preliminary pharmacological evidence suggesting a possible mechanism. Robust clinical validation is absent, and the current evidence rating is low.
