Paeoniflorin is a principal bioactive compound found in peony root, a key herb in Traditional Chinese Medicine (TCM). Its use for dizziness is primarily justified by tradition rather than robust scientific validation. In TCM, dizziness is often attributed to “Liver Yang Rising” or “blood deficiency,” and peony root (Paeonia lactiflora) is commonly included in herbal formulas to nourish blood and calm the liver, thereby alleviating symptoms like dizziness and vertigo. Historical texts, such as the “Shang Han Lun,” reference the use of peony-containing formulas for such complaints. Modern pharmacological studies have shown that paeoniflorin possesses anti-inflammatory, neuroprotective, and vasodilatory effects, but direct clinical evidence supporting its efficacy for dizziness in humans is limited. Some animal studies suggest potential benefits in models of cerebral ischemia and vestibular dysfunction, which could be related to mechanisms underlying dizziness, but these findings have not been robustly translated to clinical practice. Thus, while paeoniflorin is traditionally used for dizziness as part of multi-herb formulas, high-quality clinical trials specifically evaluating its effects on dizziness are lacking. The evidence is therefore mostly traditional, with only a low level (rated 2/5) of supporting data from scientific investigations.
