Montmorillonite, a natural clay mineral, has been used as an adsorbent agent in gastrointestinal disorders, including dyspepsia. Its primary mechanism involves adsorbing toxins, gases, and irritants in the gastrointestinal tract, thereby potentially reducing symptoms such as bloating and discomfort. Several clinical studies—mostly conducted in Europe—have evaluated the efficacy of montmorillonite (often in the form of diosmectite, a purified form) in treating various forms of digestive upset, including functional dyspepsia and diarrhea. While most robust data supports its use in acute diarrhea, a limited number of studies suggest a modest benefit in reducing symptoms of dyspepsia, such as abdominal pain and discomfort, particularly in functional gastrointestinal disorders.

The overall quality of evidence is moderate to low due to small sample sizes, variability in study design, and, in some cases, lack of placebo controls. Regulatory agencies in some countries (e.g., France) recognize montmorillonite as a symptomatic treatment for digestive discomfort, lending additional support for its use. Nevertheless, the evidence does not reach the level of support seen for standard treatments such as antacids or prokinetics. As such, montmorillonite may be considered as an adjunct or alternative in specific cases where conventional therapies are unsuitable, but more large-scale, high-quality trials are needed to firmly establish its efficacy for dyspepsia.
