Synopsis

Source of validity: Traditional
Rating (out of 5): 1

Betel (Piper betle) leaves have a long history of use in traditional medicine, particularly in South and Southeast Asia. In these systems, betel leaves are sometimes used as a remedy for ear infections and earache. Traditional methods include warming a betel leaf and placing it over the affected ear, or instilling juice extracted from the leaves into the ear canal. The rationale is based on the antimicrobial and anti-inflammatory properties attributed to betel leaves in Ayurveda and folk medicine. There is some in vitro research suggesting that betel leaf extracts exhibit antibacterial activity against certain pathogens, but there is no robust clinical or pharmacological evidence specifically supporting the efficacy of betel for ear infections in humans. The use is thus primarily justified by tradition, with limited scientific exploration. No high-quality studies or clinical trials have evaluated its safety or efficacy when used for otitis media or other ear conditions, and instillation of herbal extracts in the ear is generally not recommended by modern medical practitioners due to the risk of irritation or worsening infection. In summary, while betel's use for earache is well-rooted in traditional practices, scientific validation is lacking, and the evidence is weak (rated 1/5).
