Guava leaves and extracts have been used in traditional medicine in various cultures for the management of ear infections and earaches. Folk remedies from countries such as India and parts of Southeast Asia sometimes include the topical application of guava leaf juice or warmed guava leaves to the affected ear, with the belief that guava’s anti-inflammatory and antimicrobial properties may help alleviate symptoms. The rationale is often based on the known antimicrobial activity of guava leaves against certain bacteria, as demonstrated in laboratory studies. However, these studies are mostly in vitro (test tube or petri dish), and there is a lack of direct clinical evidence evaluating the efficacy or safety of guava for ear infections in humans. No large-scale or well-conducted clinical trials support its use specifically for otitis media or otitis externa. Therefore, while the use of guava for earache or ear infection is rooted in tradition, scientific validation is minimal, and the quality of the evidence is low.
