Anthocyanins are a class of flavonoid compounds responsible for the red, purple, and blue colors in many fruits and vegetables, such as berries, grapes, and red cabbage. Their potential use in supporting or treating edema is primarily based on scientific evidence, though the strength of this evidence is modest (rated 2/5). The rationale for anthocyanins in edema management centers around their antioxidant and anti-inflammatory properties. Edema, which is the accumulation of fluid in tissues, often results from inflammation and increased capillary permeability.
Several preclinical studies have shown that anthocyanins can reduce inflammation and strengthen capillary walls, theoretically reducing fluid leakage into tissues. Some clinical studies, particularly with extracts from bilberry (Vaccinium myrtillus), which is rich in anthocyanins, have suggested benefits in improving symptoms of chronic venous insufficiency—a common cause of edema in the legs. These studies report improved microcirculation and reduced swelling. However, high-quality, large randomized controlled trials specifically targeting edema as a primary outcome are limited.
Despite these promising findings, the current scientific consensus is that anthocyanins may offer supportive benefits for edema, particularly when related to venous insufficiency, but they should not be considered a primary treatment. More robust clinical trials are necessary to confirm their efficacy and clarify dosing. Thus, while there is a scientific basis for their use, the supporting evidence remains moderate and not definitive.
