Enzyme blends, often comprising proteolytic enzymes such as bromelain, trypsin, and chymotrypsin, have been explored for their role in managing edema, particularly post-traumatic or post-surgical edema. The scientific rationale is based on these enzymes’ ability to break down inflammatory mediators and fibrin, which accumulate in tissues during edema, thereby potentially accelerating the resolution of swelling and promoting tissue healing.
Several clinical studies, mainly from Europe and Asia, have investigated oral systemic enzyme therapy for edema associated with injuries or surgery. Some randomized controlled trials and meta-analyses report modest reductions in edema and improved recovery times compared to placebo, especially in conditions like sports injuries or after dental surgery. However, the overall quality of evidence is limited by small sample sizes, inconsistent methodology, and potential publication bias.
Authoritative reviews, such as those from Cochrane, note that while proteolytic enzymes may have some anti-inflammatory and anti-edematous effects, the evidence is not robust enough to make strong clinical recommendations. Their use is not widely endorsed in major clinical guidelines for edema.
