Hesperidin is a flavonoid primarily found in citrus fruits and is commonly used in combination with diosmin to support vascular health. Its use for edema, particularly chronic venous insufficiency (CVI) and related forms of lower limb swelling, is supported by several clinical studies and meta-analyses. Hesperidin, often as micronized purified flavonoid fraction (MPFF), appears to improve venous tone, reduce capillary permeability, and inhibit inflammatory mediators that contribute to edema formation in venous disorders.
Multiple randomized controlled trials and systematic reviews have reported that MPFF (usually a combination of diosmin and hesperidin) can reduce symptoms of CVI, including edema, heaviness, and leg pain. For example, a 2012 Cochrane review concluded that flavonoids may have a significant effect in reducing leg edema in patients with CVI compared to placebo. However, most studies use hesperidin in combination with diosmin, making it difficult to attribute the effect solely to hesperidin.
The evidence for hesperidin in edema not related to venous insufficiency (such as systemic or cardiac edema) is lacking. The majority of scientific support is for hesperidin’s use in venous edema, and while the evidence is moderate and generally positive, it does not reach the highest level of clinical validation, nor is hesperidin considered a first-line therapy. Nonetheless, its use is justified by clinical research, especially as part of MPFF, with a moderate strength of evidence.
