Pineapple has been explored for its potential to support or treat edema primarily due to its content of bromelain, a mixture of proteolytic enzymes found in the fruit and stem. Bromelain is believed to have anti-inflammatory, anti-edematous, and fibrinolytic properties. Several small clinical studies and animal models have suggested that bromelain may reduce swelling and bruising after surgery or injury, which are forms of localized edema. For example, bromelain supplementation has been used post-operatively in dental, nasal, and orthopedic surgeries to decrease swelling and improve recovery times. The mechanism is thought to involve modulation of prostaglandin levels, reduction of inflammatory mediators, and breakdown of fibrin, thereby enhancing reabsorption of edema fluid.

However, the evidence is limited and often of low methodological quality. Most studies have small sample sizes, lack rigorous controls, or investigate only short-term, post-surgical edema rather than chronic or systemic causes of edema (e.g., heart failure, kidney disease). Systematic reviews generally conclude that while bromelain appears safe and may be modestly effective in select cases, more robust, large-scale clinical trials are needed before it can be recommended as a standard treatment for edema.

Traditional medical systems, such as Ayurveda and Traditional Chinese Medicine, have not specifically highlighted pineapple for edema, though the fruit is used for digestive and anti-inflammatory purposes. Thus, while there is some scientific plausibility and preliminary evidence, the use of pineapple/bromelain for edema is not strongly established and should be approached with caution.
