Trypsin, a proteolytic enzyme, has been used in medical practice for its anti-inflammatory and debriding properties. Its use for edema, particularly in cases of soft tissue injury or post-surgical swelling, is based on its ability to break down proteins involved in inflammation and facilitate the removal of necrotic tissue and inflammatory exudates. Scientifically, trypsin has been studied primarily in combination with other proteolytic enzymes (such as chymotrypsin or bromelain) for the management of post-traumatic and post-operative edema. Some clinical trials from the mid to late 20th century reported modest reductions in swelling and improved tissue healing when enzyme therapy was used adjunctively. The proposed mechanism involves trypsin’s ability to hydrolyze fibrin and other proteins that contribute to inflammatory edema, thus enhancing lymphatic drainage and reducing fluid accumulation. However, evidence is mixed and contemporary systematic reviews have noted a lack of large, high-quality randomized controlled trials (RCTs) to definitively support trypsin’s efficacy for edema. Most supportive studies are small, dated, or lack rigorous methodology. Furthermore, the use of trypsin for edema is less common in modern Western medicine and is not included in major guidelines for edema management. While scientific rationale exists and some evidence supports its use, the overall quality and quantity of evidence is limited, justifying a moderate-low evidence rating.
