Belleric myrobalan (Terminalia bellirica), known as "Bibhitaki" in Ayurveda, is traditionally used in South Asian herbal medicine for various respiratory issues, including cough, bronchitis, and asthma. In classic Ayurvedic formulations, especially "Triphala," Belleric myrobalan is attributed with expectorant and anti-inflammatory properties, and it is described as beneficial for clearing mucus from the respiratory tract. However, there is little direct mention of its use for emphysema—a chronic and specific form of chronic obstructive pulmonary disease (COPD) characterized by alveolar damage—in traditional texts. Instead, its use is generally for broader respiratory health and not specifically for emphysema. Scientific studies on Belleric myrobalan’s effects on emphysema are extremely limited. While some laboratory and animal studies have shown its extracts possess anti-inflammatory and antioxidant properties, these findings do not directly translate to clinical evidence for emphysema treatment. No high-quality clinical trials have demonstrated efficacy or safety for this particular condition. Therefore, while the use of Belleric myrobalan for respiratory support has traditional precedent, its specific application to emphysema is not strongly justified by either tradition or science, and evidence supporting its use in this context is weak.
