Boswellia serrata is an herbal extract traditionally used in Ayurvedic medicine for inflammatory conditions. Its main active compounds, boswellic acids, have demonstrated anti-inflammatory properties by inhibiting 5-lipoxygenase, reducing leukotriene synthesis, and modulating cytokine production. These mechanisms are relevant to endometriosis, a condition characterized by chronic inflammation and aberrant immune responses.

There is preclinical evidence supporting Boswellia's potential role in endometriosis. A few animal studies have shown that Boswellia serrata extracts can decrease the size of endometriotic lesions and reduce inflammatory markers in rodent models of endometriosis. For example, a 2011 study in rats (Kamatou et al.) demonstrated that Boswellia extract reduced lesion volume and improved histological appearance.

However, clinical evidence in humans is lacking. No randomized controlled trials have specifically evaluated Boswellia serrata as a treatment for endometriosis in women. Its use is therefore based on extrapolation from its anti-inflammatory effects in other conditions (such as osteoarthritis) and from limited animal data. While these findings are promising, more rigorous human studies are needed to establish efficacy and safety in endometriosis management. Thus, the evidence rating is 2/5: preliminary experimental support, but no direct clinical validation.
