Boswellic acid, a major active component of the resin from the Boswellia serrata tree (frankincense), is known for its anti-inflammatory properties, largely via the inhibition of 5-lipoxygenase and reduction of leukotriene synthesis. These mechanisms have prompted investigation into its use for various inflammatory conditions, including endometriosis. There is some scientific interest and limited preliminary research suggesting that boswellic acid may help manage endometriosis-related inflammation and pain. A few animal studies have shown that Boswellia extracts can reduce endometriotic lesion size and modulate inflammatory markers in induced models of endometriosis. For example, a 2019 study in rats demonstrated that Boswellia serrata extract reduced lesion area and decreased inflammatory cytokines. However, direct evidence in humans is lacking; to date, there are no well-designed clinical trials evaluating boswellic acid specifically for endometriosis. Most evidence is extrapolated from its general anti-inflammatory effects and from studies on other gynecological or inflammatory disorders. Thus, while there is a biologically plausible rationale and some supportive preclinical data, the overall evidence base is weak and does not yet justify widespread clinical use for endometriosis. More robust human studies are needed to confirm efficacy and safety in this context.
