Sulbutiamine is a synthetic derivative of thiamine (vitamin B1) developed in Japan to treat asthenia (abnormal physical weakness or lack of energy). Its use to support energy levels is primarily justified by limited scientific evidence. Several small clinical trials have investigated sulbutiamine for asthenia and fatigue, especially in post-infectious or chronic conditions. Some studies, such as a randomized, double-blind, placebo-controlled trial, have reported modest improvements in fatigue and energy levels among patients with infections or chronic fatigue syndromes. The proposed mechanism involves enhanced penetration of the blood-brain barrier compared to thiamine, potentially increasing thiamine levels in the brain and supporting neuronal energy metabolism. However, the quality of evidence is low to moderate: most studies are small, short-term, and sometimes lack rigorous controls. Sulbutiamine is approved in some countries as a treatment for fatigue, particularly post-infectious asthenia, but not as a general stimulant or energy booster. There is no robust evidence from large-scale or long-term studies to confirm significant benefits in healthy individuals or those with non-specific fatigue. Therefore, while there is some scientific validation, the evidence is limited and not conclusive. Use for energy support remains somewhat experimental, though it is better established than traditional or anecdotal use.
