Cannabidivarin (CBDV) is a non-psychoactive cannabinoid found in Cannabis sativa. Scientific interest in CBDV for treating epilepsy, particularly drug-resistant forms, has grown due to its structural similarity to cannabidiol (CBD), which is already approved for certain epileptic disorders (e.g., Dravet syndrome and Lennox-Gastaut syndrome). Preclinical research has shown that CBDV exerts anticonvulsant effects in various animal models of epilepsy, including chemically and genetically induced seizure models. These studies suggest that CBDV may modulate neuronal hyperexcitability through mechanisms such as interacting with transient receptor potential (TRP) channels and inhibiting abnormal neuronal firing.
Clinical evidence, however, remains limited. A phase IIa randomized, double-blind, placebo-controlled trial of GWP42006 (a pharmaceutical-grade CBDV formulation) in adult patients with focal seizures did not meet its primary efficacy endpoint, although the drug was well-tolerated. Some secondary analyses and preclinical data continue to suggest potential, particularly in pediatric populations and rare epileptic syndromes. Overall, while the biological plausibility and preclinical efficacy are strong, robust clinical data supporting the effectiveness of CBDV in epilepsy are still lacking. Thus, CBDV is considered a promising investigational compound, but it is not yet an established or approved therapy for epilepsy.
