Higenamine is a naturally occurring plant alkaloid found in several traditional medicinal plants, such as Nandina domestica and Aconitum species. In recent years, it has been included in some dietary supplements marketed for exercise performance, weight loss, and fat burning. The rationale for its use in exercise contexts is largely based on its role as a beta-adrenergic agonist, which theoretically could stimulate cardiac output, increase energy expenditure, and promote fat breakdown. However, scientific evidence supporting these claims is very limited. Most of the research available consists of preclinical studies or small-scale human trials that often lack rigorous controls. A few studies have demonstrated that higenamine can increase heart rate and may have mild stimulant properties, but there is insufficient evidence to conclude that it enhances exercise performance or significantly aids fat loss in healthy individuals. Moreover, concerns have been raised regarding its safety and potential cardiovascular risks, leading to its inclusion on the World Anti-Doping Agency (WADA) prohibited list. In summary, while there is a plausible pharmacological mechanism and some very limited human data, the quality and quantity of evidence do not robustly support the use of higenamine for exercise performance or fitness enhancement.
