Alpha-linolenic acid (ALA) is an omega-3 fatty acid found in plant sources such as flaxseed, chia seeds, and walnuts. Its use in supporting or treating Fatty Liver Disease (non-alcoholic fatty liver disease, NAFLD) is primarily based on emerging scientific evidence, though the strength and quality of this evidence are limited, warranting a rating of 2 out of 5.

Preclinical studies have demonstrated that ALA supplementation may help reduce liver fat accumulation, inflammation, and oxidative stress in animal models of NAFLD. Proposed mechanisms include the modulation of lipid metabolism, reduction of hepatic triglyceride synthesis, and improvement in insulin sensitivity. Some small-scale human studies and clinical trials suggest that diets higher in ALA may be associated with lower markers of liver fat and inflammation. However, most human studies are observational, have small sample sizes, or combine ALA with other nutrients, making it difficult to attribute observed benefits solely to ALA.

Current clinical guidelines for NAFLD focus on lifestyle interventions such as weight loss, diet, and exercise, and while omega-3 fatty acids (especially EPA/DHA from fish oil) have more robust evidence, ALA's role is still under investigation. There is insufficient high-quality evidence to recommend ALA supplementation as a standard treatment for fatty liver disease, but it remains an area of active research. Individuals interested in ALA for liver health should consult healthcare professionals, as more definitive human studies are needed.
