Astragaloside, a key bioactive saponin compound derived from Astragalus membranaceus, has been investigated in recent years for its potential role in managing fatty liver disease (non-alcoholic fatty liver disease, NAFLD). Preclinical studies in animal models and cell cultures have demonstrated that astragaloside may exert hepatoprotective effects by modulating lipid metabolism, reducing hepatic steatosis, attenuating oxidative stress, and inhibiting inflammatory pathways associated with NAFLD. For example, several studies have indicated that astragaloside IV can activate AMP-activated protein kinase (AMPK), thereby reducing lipid accumulation in liver cells, and suppress the expression of pro-inflammatory cytokines such as TNF-α and IL-6.
However, despite these promising laboratory findings, there is a lack of high-quality clinical trials in humans confirming the efficacy of astragaloside for fatty liver disease. Most available evidence is limited to animal studies and mechanistic research, with only preliminary or indirect evidence from small-scale human studies using Astragalus extracts (not isolated astragaloside). Consequently, while there is a scientific rationale and some experimental validation for its use, the overall quality and quantity of evidence supporting astragaloside as a treatment or supportive agent for fatty liver disease remain limited. More rigorous, large-scale human clinical trials are necessary to establish its therapeutic role and safety profile for this condition.
