Bacillus subtilis natto, a probiotic bacterium used in the fermentation of natto (a traditional Japanese food), has been investigated for its potential benefits in metabolic and liver health, including fatty liver disease (non-alcoholic fatty liver disease, NAFLD). Limited preclinical studies suggest that supplementation with B. subtilis natto or natto extract may influence lipid metabolism, reduce hepatic fat accumulation, and decrease liver inflammation in animal models of fatty liver disease. The proposed mechanisms include modulation of gut microbiota, anti-inflammatory effects, and improvements in insulin sensitivity. For example, a handful of mouse studies have found improved liver histology and reduced markers of liver injury following administration of B. subtilis natto or related fermented products.
However, robust clinical evidence in humans is lacking. There are very few, if any, high-quality randomized controlled trials directly assessing the effects of B. subtilis natto supplementation on fatty liver disease in humans. Most of the supportive evidence is preclinical or extrapolated from broader research on probiotics and liver health.
In summary, while there is a scientific rationale and some encouraging preclinical data, the overall quality and quantity of evidence supporting B. subtilis natto for the treatment or support of fatty liver disease is currently low. More well-designed human studies are needed to establish efficacy and safety.
