Barberry (Berberis vulgaris) has been traditionally used in various cultures for liver-related ailments, but recent scientific research provides moderate evidence supporting its efficacy in managing Non-Alcoholic Fatty Liver Disease (NAFLD). The primary active compound in barberry is berberine, an isoquinoline alkaloid known for its anti-inflammatory, antioxidant, and metabolic regulatory properties. Several clinical trials and animal studies have investigated the effects of berberine on liver health, particularly in the context of fatty liver disease.

A number of randomized controlled trials have demonstrated that supplementation with barberry extract or berberine can improve biomarkers associated with NAFLD, including reductions in liver fat content, serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels, and improvements in lipid profiles and insulin sensitivity. For example, a 2020 double-blind, placebo-controlled clinical trial showed significant improvements in liver enzymes and hepatic steatosis in NAFLD patients treated with barberry extract. The proposed mechanisms include berberine’s ability to modulate lipid metabolism, reduce oxidative stress, and decrease inflammation in hepatic tissue.

While promising, the body of evidence is still developing and more high-quality, large-scale studies are needed to confirm long-term safety and efficacy. Nevertheless, the current scientific literature provides moderate support for the use of barberry as an adjunctive treatment for fatty liver disease, especially when combined with lifestyle interventions.
