Bifidobacterium species have been studied for their potential role in supporting or treating Nonalcoholic Fatty Liver Disease (NAFLD) and related liver conditions. The rationale is based on evidence that gut microbiota play a significant role in liver health via the gut-liver axis. Dysbiosis, or an imbalance in gut microbiota, is frequently observed in patients with fatty liver disease. Probiotic supplementation with Bifidobacterium may help restore microbial balance, reduce systemic inflammation, and improve metabolic parameters.
Several small-scale randomized controlled trials (RCTs) and meta-analyses have evaluated the impact of Bifidobacterium-containing probiotics on fatty liver disease markers. These studies have demonstrated improvements in liver enzymes (such as ALT and AST), reductions in liver fat content (measured by imaging or biomarkers), and improvements in insulin resistance. Mechanistically, Bifidobacterium may exert beneficial effects by strengthening intestinal barrier integrity, reducing endotoxin translocation, and modulating inflammatory pathways.
While these findings are promising, the methodological quality and sample sizes of existing studies vary, and most use multi-strain probiotic formulations rather than Bifidobacterium alone. Therefore, evidence supporting Bifidobacterium for fatty liver disease can be rated as moderate (3/5), indicating scientific validation but with a need for further large, high-quality RCTs to confirm efficacy and optimal dosing.
