Fatty acid or fat/oil supplements, particularly those containing omega-3 polyunsaturated fatty acids (PUFAs) such as eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), have been studied for their potential benefits in treating nonalcoholic fatty liver disease (NAFLD). Several randomized controlled trials and meta-analyses have indicated that supplementation with omega-3 fatty acids can reduce liver fat content and improve certain markers of liver health, such as alanine aminotransferase (ALT) and triglyceride levels, in patients with NAFLD. The mechanism is thought to involve the modulation of lipid metabolism, reduction of hepatic inflammation, and improvement in insulin sensitivity.
However, the evidence is mixed regarding the impact of fatty acid supplementation on more advanced liver disease or fibrosis. The American Association for the Study of Liver Diseases (AASLD) acknowledges that omega-3 fatty acids may be considered as a treatment for hypertriglyceridemia in patients with NAFLD but does not recommend them as a primary therapy for NAFLD itself. Other types of fat or oil supplements (such as saturated fats or blends not specifically high in omega-3s) have not shown benefit and may even be harmful.
Overall, the scientific evidence supports a moderate benefit for certain fatty acid supplements (notably omega-3s) in reducing liver fat in NAFLD, with further research needed to clarify their role in advanced liver disease.
