Synopsis

Source of validity: Scientific
Rating (out of 5): 3

DHA (docosahexaenoic acid) is an omega-3 fatty acid found primarily in fish oil. Its use in supporting or treating Nonalcoholic Fatty Liver Disease (NAFLD) is backed by a moderate but growing body of scientific evidence. Several clinical studies and animal experiments have suggested that DHA supplementation can reduce liver fat content, improve liver enzyme levels (such as ALT and AST), and decrease markers of inflammation and fibrosis in patients with NAFLD. The likely mechanisms include DHA’s anti-inflammatory effects, its ability to modulate lipid metabolism, and its impact on insulin sensitivity.

A 2015 randomized controlled trial (Scorletti et al., Gastroenterology) found that high-dose omega-3 supplementation (including DHA) reduced liver fat in NAFLD patients. Other meta-analyses and systematic reviews (notably, Parker et al., 2012; Yan et al., 2022) have reported modest but significant improvements in liver fat and biochemical markers. However, outcomes are variable, and some studies show limited benefit, especially in advanced disease stages.

Guidelines from some professional organizations acknowledge that omega-3 fatty acids (including DHA) may have a role in NAFLD management, but they are not considered a stand-alone therapy. More large-scale, long-term studies are needed for definitive conclusions. In summary, there is scientific evidence supporting DHA's benefit for fatty liver disease, but it is not yet universally endorsed as a primary treatment.
