Ethyl esters, specifically omega-3 fatty acid ethyl esters such as EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid), are used to support or treat fatty liver disease, particularly non-alcoholic fatty liver disease (NAFLD). The rationale for their use stems from the anti-inflammatory effects of omega-3 fatty acids, their ability to reduce hepatic triglyceride accumulation, and their favorable influence on lipid metabolism. Several randomized controlled trials and meta-analyses have investigated the efficacy of omega-3 ethyl esters in NAFLD. Results generally show that supplementation can reduce liver fat content and improve liver enzyme levels (such as ALT and AST), though effects on fibrosis and histology are less clear. For example, a 2020 meta-analysis published in Clinical Nutrition found that omega-3 supplementation led to significant reductions in liver fat and improvements in some markers of liver function in patients with NAFLD. However, most studies are moderate in size and duration, and some report only modest benefit. Guidelines from major liver societies recognize omega-3 fatty acids as a possible adjunctive therapy but do not recommend them as first-line treatment. Thus, while there is scientific validation for the use of omega-3 ethyl esters in supporting fatty liver disease, the evidence is moderate and further large, long-term trials are needed to better define their role.
