Fish, particularly fatty fish (such as salmon, sardines, and mackerel), and to a lesser extent shellfish, are supported by scientific evidence for their beneficial effects in nonalcoholic fatty liver disease (NAFLD). The main rationale lies in their high content of omega-3 polyunsaturated fatty acids (PUFAs), which have been shown in multiple clinical studies and meta-analyses to reduce hepatic fat accumulation, inflammation, and improve metabolic parameters associated with NAFLD. Omega-3 PUFAs may decrease liver fat by modulating lipid metabolism, reducing de novo lipogenesis, enhancing fatty acid oxidation, and exerting anti-inflammatory effects. Randomized controlled trials have demonstrated that supplementation with fish oil or increased dietary intake of fish can lead to significant reductions in liver fat content, liver enzyme levels, and some histological features of NAFLD. Shellfish, while lower in omega-3s than fatty fish, are still a source of high-quality protein and micronutrients that may indirectly benefit liver health, although the evidence for shellfish specifically in NAFLD is less robust. Leading hepatology guidelines (e.g., American Association for the Study of Liver Diseases) recommend increased consumption of fish, especially those rich in omega-3s, as part of lifestyle interventions for NAFLD. However, while the evidence is strong for fatty fish and omega-3s (hence the rating of 4), fish or shellfish alone are not a standalone treatment, but rather a component of broader dietary and lifestyle management.
