Fish oil, rich in omega-3 polyunsaturated fatty acids (PUFAs) such as eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), has been studied for its potential benefits in managing non-alcoholic fatty liver disease (NAFLD). The scientific rationale is based on omega-3’s ability to modulate lipid metabolism, reduce hepatic triglyceride accumulation, and exert anti-inflammatory effects. Several randomized controlled trials and meta-analyses suggest that fish oil supplementation can reduce liver fat content and improve liver enzyme levels in patients with NAFLD. For instance, a 2016 meta-analysis in "PLOS One" concluded that omega-3 supplementation is associated with decreased liver fat and improvements in ALT and AST levels.
However, while the bulk of evidence supports modest benefit, results are not entirely consistent across all studies, with some trials showing no significant effect on hepatic steatosis or fibrosis. Most guidelines do not yet recommend fish oil as a standard therapy, but they do acknowledge its potential as an adjunct to lifestyle interventions. The overall evidence is moderate (rated 3/5), with more rigorous and larger trials needed to clarify its efficacy and optimal dosage. In summary, fish oil is scientifically investigated and shows promise for fatty liver disease, but it should not replace primary treatments such as diet and exercise.
