Flavonolignans, particularly those found in silymarin (the active extract of Silybum marianum or milk thistle), have been investigated for their effects on fatty liver disease, including non-alcoholic fatty liver disease (NAFLD). Several clinical studies and meta-analyses suggest that silymarin, rich in flavonolignans such as silybin, may exert hepatoprotective effects. Proposed mechanisms include antioxidant activity, anti-inflammatory effects, and improved insulin sensitivity, all of which are relevant to the pathogenesis of fatty liver disease.
A number of randomized controlled trials have reported modest improvements in liver enzyme levels (such as ALT and AST) and some evidence of improved liver histology in patients with NAFLD or alcoholic fatty liver disease when treated with silymarin-based preparations. However, the magnitude of benefit is typically small to moderate, and not all studies agree on clinically significant outcomes. Safety profiles are generally favorable.
Although silymarin and its flavonolignans have a long history of traditional use for liver ailments, contemporary support for their use in fatty liver disease is primarily scientific, with evidence suggesting potential but still somewhat limited efficacy. The overall quality of evidence is moderate (rated 3/5), reflecting a combination of positive but not definitive trial outcomes, heterogeneity among study designs, and a need for larger, more rigorous trials.
