GW501516 (also known as Cardarine) is a selective PPARδ agonist that has been investigated in preclinical and limited clinical studies for its metabolic effects. There is some scientific rationale for its potential use in treating non-alcoholic fatty liver disease (NAFLD) and related metabolic disorders. Animal studies have shown that GW501516 can improve lipid metabolism, decrease hepatic triglyceride accumulation, and enhance fatty acid oxidation, which theoretically could benefit patients with fatty liver disease. For example, studies in mice have demonstrated reduced hepatic steatosis and improved insulin sensitivity with GW501516 treatment. Some research also indicates that PPARδ agonists can modulate inflammation and fibrosis pathways in the liver.

However, the evidence supporting its use in humans for NAFLD is extremely limited. There are no large-scale randomized controlled trials demonstrating safety and efficacy of GW501516 in patients with fatty liver disease. Moreover, concerns about carcinogenicity observed in long-term animal studies have halted its clinical development for metabolic diseases. As a result, GW501516 is not approved for medical use, and its safety profile in humans remains uncertain.

In summary, while there is a mechanistic and preclinical scientific basis for considering GW501516 in the context of fatty liver disease, the current level of clinical evidence is low, and use in humans is not justified outside of a research setting.
