Hydroxycinnamic acids are a class of phenolic compounds found in many plant-based foods, including coffee, whole grains, fruits, and vegetables. The most common hydroxycinnamic acids are caffeic acid, ferulic acid, and p-coumaric acid. There is emerging scientific interest in these compounds for their antioxidant and anti-inflammatory properties, which may be relevant to metabolic disorders such as non-alcoholic fatty liver disease (NAFLD).

Preclinical studies (mainly in vitro and animal models) suggest that hydroxycinnamic acids may attenuate hepatic steatosis, reduce liver inflammation, and improve oxidative stress markers, all of which are implicated in the progression of fatty liver disease. For example, ferulic acid has been shown in rodent studies to improve lipid metabolism and decrease fat accumulation in the liver. Caffeic acid also appears to modulate molecular pathways involved in lipid metabolism and inflammation.

However, direct evidence from human clinical trials is limited. Most studies to date are small, preliminary, or use dietary sources (such as coffee) rather than isolated hydroxycinnamic acids, making it difficult to attribute the effects solely to these compounds. As such, while there is a scientific rationale and some supportive preclinical evidence, robust clinical validation for the use of hydroxycinnamic acid in treating fatty liver disease is lacking at this time.
