Isosilybin is one of the flavonolignans found in silymarin, the active extract from the seeds of Silybum marianum (milk thistle). Silymarin and its individual components, including isosilybin, have a long history of use in traditional medicine for liver ailments. However, in recent years, there has been growing scientific interest in their potential to support or treat fatty liver disease (non-alcoholic fatty liver disease, NAFLD). Preclinical studies (in vitro and animal models) have demonstrated that isosilybin can exert antioxidant, anti-inflammatory, and antifibrotic effects, which may be beneficial for liver health. Some research indicates that isosilybin may reduce lipid accumulation in liver cells, modulate insulin resistance, and protect hepatocytes from oxidative stress.
Despite promising preclinical data, direct clinical evidence for isosilybin alone in the treatment of fatty liver disease in humans is limited. Most human studies have investigated silymarin as a whole rather than isolated isosilybin, and results often show modest improvements in liver enzymes and some markers of liver health. A few small clinical studies and pilot trials have suggested potential benefits of silymarin or its enriched fractions (containing isosilybin) in NAFLD, but these are not yet conclusive. As such, while there is a scientific rationale and preliminary evidence for isosilybin's role, robust, large-scale human trials specifically investigating isosilybin in fatty liver disease are lacking.
