L-Glutathione is an endogenous antioxidant that plays a key role in cellular detoxification and protection against oxidative stress. In the context of Fatty Liver Disease (both NAFLD and alcoholic fatty liver), oxidative stress is a major contributor to liver cell injury and disease progression. Several scientific studies, including small clinical trials and animal studies, have investigated L-glutathione’s potential in supporting liver health. Some clinical evidence suggests that intravenous or oral glutathione may improve liver function tests (e.g., ALT, AST) and decrease markers of oxidative stress in patients with fatty liver disease, particularly those with nonalcoholic steatohepatitis (NASH) or alcoholic liver disease.
A notable open-label pilot study (2017, Journal of Gastroenterology and Hepatology) found that oral glutathione supplementation (300 mg/day for 4 months) significantly reduced ALT levels and improved hepatic steatosis in NAFLD patients. However, these findings are preliminary, and larger, controlled trials are needed to confirm efficacy. Mechanistically, glutathione's antioxidant properties and its involvement in modulating inflammatory and fibrotic pathways provide a plausible rationale for its use.
Despite promising early data, the overall quality and quantity of evidence remain moderate, and glutathione is not yet a standard therapy for fatty liver disease. Its use is primarily justified by mechanistic rationale and preliminary clinical data, rather than robust, large-scale trials or long-standing tradition.
