Lecithin, a mixture of phospholipids (primarily phosphatidylcholine), has been investigated for its potential benefits in supporting or treating Fatty Liver Disease (FLD), particularly non-alcoholic fatty liver disease (NAFLD). The rationale behind its use is based on the role of phosphatidylcholine in hepatic lipid metabolism and the formation and export of very-low-density lipoproteins (VLDL), which help transport triglycerides out of the liver. Some animal studies and small human trials have suggested that lecithin supplementation may improve liver function by enhancing lipid export and reducing hepatic fat accumulation. For example, studies in rodents have shown that diets enriched with phosphatidylcholine can attenuate liver fat accumulation and improve markers of liver health. However, robust clinical evidence in humans is limited. A few small clinical studies and reviews have indicated possible improvements in liver enzyme levels and hepatic steatosis, but the studies often suffer from methodological limitations such as small sample sizes, lack of controls, or short duration. Overall, while there is a plausible biochemical mechanism and some preliminary evidence supporting lecithin’s use in fatty liver disease, large-scale, well-designed clinical trials are lacking. Thus, scientific support exists but is currently weak to moderate, and lecithin cannot yet be recommended as a primary therapy for FLD. Its use is best viewed as adjunctive, pending further research.
